Why? Clips that attach the tent to the top of the playpen can break or be removed by a child. A child can lift the tent and become entrapped at the neck between the rigid playpen frame and the metal base rod of the tent, posing a strangulation hazard.

What models? The Cozy Indoor Outdoor Portable Playard Tents Plus Cabana Kits. The dome-shaped white-colored mesh tent is designed to fit over playpens as small as 28 inches by 40 inches or as large as 31 inches by 44 inches to contain a child. There are 12 plastic clips to secure the base of the tent to the top rail of the playpen through button holes along the bottom of the tent. The tent has a zippered side for putting in and taking out the child.

Coumadin 1 mg tablet blister packs

Company: Bristol-Myers Squibb.

Quantity: Eight lots.

Why? Some of the tablets, over time, may not meet specification for isopropanol. Isopropanol is used to maintain the active ingredient, coumadin, in the crystalline state, and could affect the therapeutic levels of the active ingredient. A decrease of active ingredient may increase the risk of clots, which could lead to heart attack or stroke. If there is too much active ingredient, there is an increased risk of bleeding.

What models? Three lots of physician sample blister packs of Coumadin 1 mg tablets and five lots of Coumadin 1 mg tablet hospital unit dose (HUD) blister packs. The following lot numbers are included in this recall: Physician Sample Blister Packs: Lot# 9A48931A, 9A48931B, 9A48931C, expiration January 2012; HUD Blister Pack: Lot# 8F34006B, 8K44272A, 8K46168A, 9F44437A and 9K58012B with expiration dates between June 2011 and November 2012.

Vialipro dietary supplement

Why? It could contain a compound that is chemically similar to the erectile dysfunction drug sildenafil. This compound could interact with nitrates found in some prescription drugs for diabetes, high blood pressure, high cholesterol or heart disease. If this compound interacts with nitrates, it could cause blood pressure to drop to dangerous levels.

What models? Vialipro with the following lot numbers: 80409, 80661, 81146, 90132, 90265, 90587, 90826, 91065, 00197 and 00347.

Why? The product have the potential to be contaminated with E. coli O157:H7. E. coli O157:H7 is a bacterium that can cause serious foodborne illness in a person who eats a food item contaminated with it.

What models? Romaine lettuce salad products with expired Use-by Dates of July 8-12 and an "S" in the product code.

Why? The gas cap can leak or detach from the fuel tank, posing a fire and burn hazard. Also, the throttle can stick due to an improperly positioned fuel line and throttle cable, posing a sudden acceleration hazard.

What models? Baja Motorsports mini bikes with model numbers beginning with HT65, MB165, WR65, MB196, DB30, WR90 and DR90 and go-carts with model numbers BB65, SD65, DN65 and TR65. The model number is located on the mini bikes' fenders and/or decorative fuel tank and on the go-carts' roll cage. They both have black plastic gas caps.

Slim-30 herb supplement

Why? The product contains N-Desmethyl sibutramine and traces of sibutramine, drugs used as an appetite suppressant for weight loss. The Food and Drug Administration has not approved this product, so its safety and effectiveness is unknown. Also, sibutramine can substantially increase blood pressure and pulse rate in some patients and might be a risk for patients with a history of coronary artery disease, congestive heart failure, arrhythmias or stroke.

What models? Slim-30 herb supplement with the code number 032009.

Wire feed welders

Why? The wire welder's torch does not have a cold contactor as erroneously stated on the packaging and instruction manual. Without it, the welder generates an electrical arc immediately upon contact with the welding material, posing a burn hazard.

What models? Vaper 90 amp flux wire feed welders with model number 41181. The wire welders are red and black. "Vaper" and "90 amp flux wire welder" can be found on the product's operation panel. The model number can be found on the top right hand corner of the package and on the first page of the users' manual.

Muddy Outdoors tree climbing sticks

Company: Muddy Outdoors.

Why? Bolts that secure the cam locks to the frame of these climbing sticks that retains the rope around the tree can break, allowing the cam locks to detach from the frame. This causes the retaining rope to detach and the climbing stick to release from the tree, posing a fall hazard.

What models? 2009 year Model 70301 Muddy Outdoors climbing stick (a 20-inch single climbing stick) and 2009 year Model 70304 Muddy Outdoors climbing stick (4 pack of 20-inch climbing sticks). The year and model number is printed on a label on the front of the main vertical frame of the climbing stick just below the two cam locks.
