Back to school: immunizations

Immunizations

Pre-K

• 4-5 doses diphtheria-tetanus-pertussis vaccine or age-appropriate tetanus vaccines

• 3 doses hepatitis B vaccine

• 3-4 doses inactivated polio vaccine

• 1 dose MMR (measles, mumps, rubella)

• 1 dose varicella vaccine (chicken pox)

• Hib immunization. Numbers of doses depend on type of vaccine and student's age.

K-grade 5

• 4-5 doses diphtheria-tetanus-pertussis vaccine or age-appropriate tetanus vaccines

• 3 doses hepatitis B vaccine

• 3-4 doses inactivated polio vaccine

• 2 doses MMR (measles, mumps, rubella)

• 2 doses varicella vaccine (chicken pox)

Kindergarten only

• A fifth dose of the polio virus is required if the fourth dose was administered prior to the fourth birthday. If the third dose was administered after the fourth birthday, a fourth dose is not required.

Grade 6

• 4-5 doses diphtheria-tetanus-pertussis vaccine or age-appropriate tetanus vaccines

• 3 doses hepatitis B vaccine

• 3-4 doses inactivated polio vaccine

• 2 doses MMR (measles, mumps, rubella)

• 1 dose varicella vaccine (chicken pox)

Grades 7-12

• 4-5 doses diphtheria-tetanus-pertussis vaccine or age-appropriate tetanus vaccines

• 2-3 doses hepatitis B vaccine

• 3-4 doses inactivated polio vaccine

• 2 doses MMR (measles, mumps, rubella)

• 1 dose varicella vaccine (chicken pox)

Grades 7-11 only

• 1 dose tetanus-diphtheria-pertussis

Grade 12 only

• 1 dose tetanus-diphtheria-pertussis or tetanus-diphtheria
