Things to know to prepare for 2013-14 school year in Hillsborough

Hillsborough County parents can find more back-to-school information at www.sdhc.k12.fl.us. Here are a few highlights.

Identifying your school

To find out which school your child is zoned for, visit the Hillsborough County public schools website at http://gis.sdhc.k12.fl.us/schoollocator. Put in your address, and the site will generate which school is appropriate for each grade level, as well as contact information. If parents have questions about their zoned school, they can also call the Choice Line: (813) 272-4692.

Registering your child

Parents must accompany their children to register at their designated school. If the child is coming from a public school outside of Florida, or from a private school anywhere, families need to provide the following:

• Proof of a physical examination in the past 12 months by an approved, licensed health care provider or the Hillsborough County Health Department.

• A report card or transcript from the last school attended (the new school's registrar will send for the permanent record).

• Verification of parent or legal guardian's address, such as a tax receipt or proof of homestead exemption, current electric bill, contract for purchase of home, warranty deed or lease agreement.

• Proof of birth date, such as a birth certificate or state of Florida birth registration card. Alternatively, the family can provide a baptismal certificate showing date of birth and place of baptism, so long as it is accompanied by parents' sworn affidavit; an insurance policy on the child in force for at least two years; a Bible record of the child's birth accompanied by the parents' sworn affidavit; a passport or certificate of arrival in the United States showing the age of the child; a school record at least four years prior, showing date of birth; or a parents' sworn affidavit accompanied by a certificate of examination from a health officer or physician verifying the child's (physical) age.

• Immunization records showing proof of proper immunization.

New to Hillsborough?

If the child is coming from another public school in Florida but outside Hillsborough, he or she will need to provide:

• A report card or transcript from the last school attended (the new school's registrar will send for the permanent record).

• Verification of parent or legal guardian's address, such as a tax receipt or proof of homestead exemption, current electric bill, contract for purchase of home, warranty deed or lease agreement.

• Proof of birth date (see above).

• Immunization records showing proof of proper immunization.

Back-to-school nights/orientations

Back-to-school nights and orientations are typically held in the second or third week after school starts, so plan for sometime in the first two weeks of September. Parents will be getting back-to-school packets in the mail with specific dates. Some schools may also put out a phone call.

Bus routes

Parents can check the district's website for their child's bus route. Visit www.sdhc.k12.fl.us/transportation/routes.asp. For choice or magnet routes, visit www.sdhc.k12.fl.us/choice.

Lunch and breakfast prices

Breakfast is free for all students. Lunch is free for all students who meet the income guidelines for free and reduced price meals.

Elementary school lunch: $2.25

Middle and high school lunch: $2.75

Pre-K

• Varicella (chicken pox) vaccine or date of disease (year) as verified by parent or physician

• 3 doses hepatitis B

• 4 doses Hib

• Up to date for age for DTaP, polio, and MMR

Kindergarten

• 5 doses DPT (diphtheria, pertussis, tetanus)

• 3-5 doses polio

• 2 doses MMR (measles, mumps, rubella)

• 3 doses hepatitis B

• 2 doses varicella (chicken pox) or have had the disease

Grades 1-5

• 5 doses DPT (diphtheria, pertussis, tetanus)

• 4 doses polio

• 2 doses MMR (measles, mumps, rubella)

• 3 doses hepatitis B

Grade 6

• 5 doses DPT (diphtheria, pertussis and tetanus)

Grades 7-12

Grades 7-11 only
