REQUIRED IMMUNIZATIONS

Grades K-1: measles vaccine (second dose, preferably MMR); hepatitis B series (three doses over a six-month period); DTP (total of four or five doses required); polio (total of three or four doses required).

Grades 2-6: measles vaccine (second dose, preferably MMR); DTP (total of four or five doses required); polio (total of three or four doses required).

Grades 7-9: tetanus-diphtheria (Td) booster; measles vaccine (second dose, preferably MMR); hepatitis B series (three doses given over a six-month period).

All immunization dates must be on the Florida Certificate of Immunization (DH 680). For immunization information, contact your private physician or an HRS Pinellas County public health unit:
